Also fixes an issue with OpenAI Responses client not setting this value on responses.

The reason for the rename is we think "conversation ID" doesn't give such a strong impression that it would be fixed, and is more amenable to a definition like "an identifier for the current state of the conversation". This is important because for some providers, the value will change with every message that gets posted.

I know we could be more explicit still and call it something like "ContinuationStateId" but anything like that feels too abstract. This is the naming tradeoff we arrived at.
