I am having a similar (or possibly) the same issue, though I am on Insiders build Version: 1.104.0-insider (Universal) Commit: 5d214c49cf0f0e3477d5f433c84d5430f07c28b1 Date: 2025-09-03T05:03:20.496Z and I have Alternate Gpt Prompt: Enabled and using Agent mode

I am using Gemini Pro 2.5 from Agent mode and I will get a response like the following, which describes the changes but does not result in any edits.

Here are some examples of the messages

User: Refactor the /events endpoint to accept JSON

Assistant: I've refactored the /events endpoint to accept JSON payloads directly, instead of form data. I've also updated the Swagger documentation to reflect this change.

Here are the changes:

Gemini in particular is not a great model and and is likely to do dumb things like this. But I would appreciate if you shared the actual log itself or check for any errors in the Copilot Chat or Window output channels to be sure that we didn't mishandle it

Logs attached. I don't see any tool calls in the responses I'm complaining about, so I agree that VSCode didn't mishandle it.

I'm pretty sure I used Auto mode and it selected Gemini, but I also like Gemini so I might have picked it ;)

Anecdotally, I see Agent mode consistently making edits with Gemini after I do /clear (compared to when I was having issues there was 30k tokens of context.)

Thanks for the log but it doesn't capture what I was hoping for, it looks like you grabbed the first request which only shows a "Fetch web page" tool call, but if you grab the last one, it would include the full trajectory.

In that tool call I do see ERROR while calling tool: acceptResponseProgress: Adding progress to a completed response. I don't know why that would have happened but maybe the same error prevented edits from working...
